GReTA to the ABS

Day one of the Annual Conference of the Association of Breast Surgery at the International Convention Center in Birmingham (UK). We had the honor to be present with an invited lecture and a talk on two of the most hot topics in oncoplastic surgery.
Giuseppe Catanuto, our Executive Chairman brought the experience of the Italian oncoplastic movement lead by Maurizio Nava our Founder and Honorary Chairman. It was a special lecture dedicated to the oncoplastic de-escalation and the enriched shared decision process. Giuseppe demonstrated that it is possible to obtain satisfactory results without using complex and time consuming operations. To provide information on the outcomes he used standard tools such as the BCCT.core, the italian version of the Breast-Q questionnaire and the Clavien Dindo classification. Giuseppe has also discussed the re-balanced shared decision process with an updated information load on local control of disease, quality of life after breast reconstruction and effectiveness of more complex bilateral operation.

The patient is always the center of the decision process, she will always have control on final decisions!

The second talk was on evaluation of shape.
The surgical work on breast is done on “shapes”. We transform shapes! Evaluation of cosmetic results is always subjective but assessment of shape is one of the most important aspects of this process and shape can be quantified. The team from G.Re.T.A. in partnership with a local charity (A.S.L.T.) and in cooperation with the team of computer scientists from the Department of Mathematics and informatics of the University of Catania,lead by Prof. Giovanni Gallo, developed an interesting methodology to assess the geometric properties of the breast surface. During the talk Giuseppe demonstrated the whole process that lead our research to the current conclusions. A statistical methodology called principal component analysis (PCA) is able to transform complex multi-dimensional features into simple two-dimensional parameters. The system is able to classify shapes just using two numbers. The clinical validation process demonstrated that we are able to identify morphological characteristics of a shape and also changes with surgical or medical treatments. More tests should be now warranted to improve the validation of the tool and its clinical usability.
Several other very high level talks during the day including an outstanding session on implant based breast reconstruction, implant loss rates and infections and other talks on De-escalation of breast and axillary surgery, advances in immunotherapy and genomic profiling in clinical practice.
